Providers that receive a “C” or below (in any category
or overall) must submit a corrective action plan to address any deficiencies.
Providers that receive a “D” or “F” in any category
or overall are placed on probation for the following year and must submit
a corrective action plan prior to providing SES services during that school
year. In addition, providers that receive an “F” in the SAME
category for two consecutive years (e.g., receiving an “F” in
customer satisfaction for two consecutive years) or receiving an “F” overall
for two consecutive years are removed from the state-approved provider
list.
